PANDUAN MENGISI BORANG PERMOHONAN

MEMBINA/MEMBUAT/MENDIRIKAN/MENYELENGGARA/MENGGUNAKAN JETI (Instruction for Completion for ApplicationTo Built/Construct/Erect/Maintain/Use of Jetty

1.      Permohonan Membina/Membuat/Mendirikan/Menyelenggara/Menggunakan Jeti ini hendaklah diisi oleh pengusaha atau pengendali mana-mana jeti di atas mana-mana rizab sungai/zon banjir/zon perlindungan/zon penampan (Person who own or operate a jetty on river reserves/flood zone/protection zone/buffer zone must fill this Application To Built/Construct/Erect/Maintain/Use of Jetty).

2.      Jeti ertinya dan termasuklah sebarang dermaga, bangunan, binaan atau sebarang diskripsi jeti yang digunakan (Jetties means and including any wharves, building, structure or any jetty description that being used).

3.      Permohonan ini hendaklah disertakan sekali bersama yuran pemprosesan sebanyak RM150.00. Bayaran hendaklah dibuat dalam bentuk cek berpalang atas nama “PEMBANGUNAN LEMBAGA URUS AIR SELANGOR” dan sila kemukakan kepada Pengarah, Lembaga Urus Air Selangor (LUAS) bersama borang permohonan. (This application must be accompanied by a processing fee of RM150.00. Payment shall be in cross cheque to "PEMBANGUNAN LEMBAGA URUS AIR SELANGOR" with complete application form).

4.      Satu borang permohonan adalah mewakili sebuah jeti (One application form is applicable for one jetty).

5.      Dokumen-dokumen berikut hendaklah diserahkan bersama (sekiranya berkenaan). (The following documents are to be submitted where applicable).

                     i.            Satu sijil salinan sah hak milik (tittle) yang dikeluarkan atau borang dokumen lain yang diperuntukkan di bawah Kanun Tanah Negara bagi membuktikan bahawa pemohon adalah pemilik berdaftar tanah di mana jeti akan dibina atau untuk menggunakan tanah tersebut bagi tujuan sebuah jeti (A certificated true copy of issue document of title or such other form of document prescribed under the Land Code to prove that the applicant is registered owner of the land upon which the jetty is to be built or to use the land for the purpose of the jetty);

                   ii.            Sekiranya Pemohon bukan pemilik tanah, kebenaran bertulis dari pemilik tanah bagi menggunakan tanah di mana jeti akan dibina atau untuk menggunakan tanah tersebut bagi tujuan sebuah jeti (If Applicant is not owner of the land, written approval of the owner to use the land upon which the jetty is to be built or to use the land for the purpose of the jetty);

                  iii.            11 set peta atau pelan menunjukkan lokasi jeti (A map or plan to show the location of the jetty);

                 iv.            11 set lengkap pelan kejuruteraan, struktur dan pembinaan jeti (A complete set of engineering, structural or architectural plan for the jetty);

                   v.            Salinan sijil daripada Jabatan Kastam berkenaan pembayaran duti import yang disahkan benar (Certified true copy of certificate from the Royal Custom Department that the Import Duties have been paid or exempted);

                 vi.            Bagi syarikat atau firma, satu senarai semua pengarah/pemilik berkepentingan/rakan kongsi syarikat atau firma tersebut (in case of corporation or firm, a list of all directors/stakeholders/partners of the corporation or firm); dan

                vii.            Dokumen-dokumen lain yang diperlukan oleh Lembaga (such other documents as may required by the Authority)
